Ottawa CHC Trans Health Program (via Centretown CHC)

CHC= Community Health Centres

The service model for the Ottawa CHC Trans Health Program (18+) has been revised in order to better serve the trans community. As of November 10th 2016 individuals on our current wait list and individuals who call our central referral number will be offered a 60-90 minute intake meeting with a counsellor to explore gender transition goals, service needs and options. Individuals will no longer be required to complete a 4-6 session hormone readiness assessment with a counsellor.

During the intake meeting with a counsellor, the following options and service pathways will be discussed:

  • Hormone start and/or referral to a medical provider for hormone therapy
  • Connection to transition supports
  • Referral to counselling supports if needed
  • Assistance in linking to a primary care provider if needed

Ottawa CHC Trans Health Youth Approach

Youth counsellors from Centretown CHC and Carlington CHC provide Trans Health Youth Services to youth ages 17 & 18 and their families. Services include gender exploration, Hormone readiness assessment, navigation and referral to medical providers for hormone therapy, and transition support.

A youth approach recognizes that gender transition occurs in the context of other significant changes in a young person’s life such as changes in family, education, and social life. Trans Health Youth Services may include the involvement of family members, and significant members of the young person’s support network or community in order to support the young person to reach their gender transition goals.

We are a family doctor's office in a rural and remote area called Barry's Bay. We offer health/medical services to LGTBQ folks and we would like to make sure our community is aware that this is a safe and positive space to come.
